Due to ongoing power issues, Southern Electric have started works to fix the mains cabling in Brookside carpark. This is taking longer than they planned leading to major carpark disruption and very limited access at Brookside Surgery.

We apologise for any inconvenience caused and please do walk, if you are able, to the surgery if you need to attend. We hope this is rectified by the end of this week. Thank you for your patience.

Changes to Long Term Condition Annual Reviews Appointments

From the 1st of April we are changing the way we do Long Term Condition Annual Reviews Appointments. You will be asked to complete the Long Term Condition Questionnaire on the Brookside website at least 3 days prior to your appointment. This provides us with up to date health information, allowing us to make better use of the time you spend with our clinicians and ensure you have the most appropriate appointment.

If you are unable to complete the online form, you can use the Long Term Condition Questionnaire on the SurgeryPOD in the Brookside Surgery waiting room. Again, please complete this at least 3 days before your appointment with one of our HCAs. Your appointment will be cancelled if you haven’t completed the form in the above time frame.
