Please complete the “New Patient Registration” questionnaire on the SurgeryPod in the waiting room at Brookside Surgery within the next month. You can attend between 8am-6pm, Mon-Fri. Allow at least 24hrs before using the Surgery POD.
What is a Patient Health Team?
Your Patient Health Team is made up of a small group of GPs and Physicians Associates who have overall responsibility for coordinating your care. Your core team works with a member of our reception team (PHT Liaison), Clinical Pharmacist and a Health and Wellbeing Advisor.

We have 7 Patient Health Teams at the practice consisting of the following staff members:
