Power Issues

We are still experiencing a power issue at our Brookside surgery. This is being dealt with but in the meantime we have a temporary generator in place to power the building. We apologise for any inconvenience this has or may cause. Please bear with us whilst we deal with this issue. Thank you.

Asthma peak flow diary

Use this service to submit your asthma peak flow diary on a regular basis.

You can use this service if you:

  • are registered at the surgery
  • have been invited to do so

Before you start

We’ll ask you for:

  • your first and last name, date of birth, sex, postcode, email and phone number
  • if applicable, the details of the person you are completing the form on behalf of

How to use your peak flow meter

  • Put the pointer back to the first line on the scale – this is usually 60
  • Stand, or sit upright (choose what’s easiest for you and always do it that way)
  • Take a deep breath
  • Make sure your mouth makes a tight seal around the mouthpiece
  • Blow as hard and fast as you can into the meter
  • Write down your score (the number next to the pointer)
  • Do this 3 times in a row so you get 3 scores – and use the highest of these scores to fill in your diary
