To whom it may concern letter

Use this service to request a “To whom it may concern” letter from the GP.

This is a chargeable service with a minimum fee of £50 to be paid in full before we process the request.

In order to process this request, you will need to complete this form.

Please allow up to 28 days from payment receipt for your request to be actioned.

Please note we are unable to provide requests for any of the following:

• Proof of address
• Power of Attorney
• Mental Capacity Assessments
• Court of protection for deprivation of liberty
• Fitness to participate in sporting activities
• Suitability for private treatment

You can use this service if you:

• are registered at the surgery

• your first and last name, date of birth, email and phone number
• if applicable, the details of the person you are completing the form on behalf of

You will need to bring photo ID when collecting your report at Brookside Surgery off Gipsy Lane.
