Self referrals

There are a number of services locally that you are able to make self referrals to.

GP referrals

To request a GP referral:

Provide as much information as you can as this will help us to process your request.

ADHD/Autism Right to Choose Referrals

If you would like to request an ADHD or Autism Right to Choose referral, please follow the link below to see the criteria and complete a questionnaire.

Private referral request

If you would like to request a private referral, please complete a private referral request form.

We are unable to provide referral letters for non-medical / cosmetic treatment requests.

Please note we can only consider the process of such a request if it has been discussed within the last 4 months with a clinician here at the practice.

Please note, once a referral letter is completed by a member of the Brookside Team, this is sent to our secretaries. Urgent referrals are completed by the end of the next working day (usually the same day). Routine referrals can take longer especially during holiday periods or unexpected sickness.
