From the 1st of April we are changing the way we do Long Term Condition Annual Reviews Appointments.
You will be asked to complete the Long Term Condition Questionnaire on the Brookside website at least 3 days prior to your appointment.

If you would like a chaperone present during your consultation please advise us when booking your appointment, or when when checking in, or at any point during your consultation.
