Consent

Everyone aged 16 or more is presumed to be competent to give consent for themselves, unless the opposite is demonstrated. Young people aged 16 and 17, and legally “competent” younger children, may therefore sign a consent form for themselves, but may like a parent to countersign as well.

Some patients may want to designate another person (e.g. their carer) to discuss their medical care with the clinician on their behalf or to collect their test results because they are unable to do so themselves. We require a consent form to be in place for this.

Therefore, please complete and submit the form below. Once it is submitted, a member of our team will call the patient to arrange a telephone consultation with their GP to complete the authorisation process.
