Appointments

Complete the Anima form so we can ensure to provide you with an appointment with the most appropriate clinician.

If we are at capacity (Anima has closed) and your issue cannot wait until tomorrow, please call 111.

Cancel or change an appointment

To cancel your appointment:
Please let us know as soon as possible if you are unable to make your appointment. We appreciate any cancellations so that we can use it for another patient who needs it.

Information:

Please ensure that you do not cancel appointments using the text message service if an interpreter has been booked for you as it is important we are notified so that we can cancel the interpreter services.

Appointments – LTC, blood test or smears

Have you been invited to book a long term condition review, smear or blood test?

You can now book these appointments via the NHS App.

If you are unable to do this, please call the surgery and our team will book you in. We offer a ‘call back’ facility, so no need to sit and wait.
To organise interpreter services for routine appointments, please provide us with 10 days notice.

Need a follow up? Or need to book a routine appointment in advance?

If you need a follow up appointment about an issue that you have already spoken to a member of our clinical team about in the last 6 weeks, then please call the Practice as you do not need to complete an Anima request.

If it has been longer than 6 weeks or your symptoms have changed, please submit an Anima request with the details.

Communication support for medical appointments

At the Practice, we recognise that some patients face challenges in understanding spoken English due to language barriers or medical reasons.
If you require communication support during your appointments, we offer the following options:

Interpreters

We are happy to arrange an interpreter for you. Alternatively, you can attend your appointment with a friend or family member who can assist with translation if you prefer.

To organise interpreter services for routine appointments, please provide us with 10 days’ notice.

British Sign Language

We are happy to organise and BSL interpreter or lip reader to support you at your appointments.

Please complete the communication support form or contact the surgery to let us know what you need.

Appointment changes

If an interpreter has been booked for you, please do not cancel appointments using the text back service. Notify us promptly so we can cancel the interpreter services. If you need to rearrange an appointment with interpreter support, please give us plenty of notice.

Attendance

Once appointments are booked, please ensure you attend them. Interpreter services are costly to the NHS, and extra time is allocated for your benefit.

Large format

If you require written communication in large format or braille please complete the communication support form or contact the surgery to let us know what you need.

Remember, effective communication is essential for your wellbeing during medical appointments. We are here to support you.

If you need help when we are closed

If you need medical help now, use NHS 111 online or call 111.

NHS 111 online is for people aged 5 and over. Call 111 if you need help for a child under 5.

Call 999 in a medical or mental health emergency. This is when someone is seriously ill or injured and their life is at risk.

Home visits

If possible please try to phone reception before 11am if you require a home visit.

You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed.

You can be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
